Individual Service and Support Team (ISST) Facilitator
Individual Service and Support Teams (ISST) are interagency teams that support families and youth ages 0-18 in Garfield County with a range of needs and concerns. Eligible families would be involved with multiple systems, have children with mental or behavioral health challenges and/or academic or developmental concerns. Teams include representatives from Garfield County Public Health and Department of Human Services, Mind Springs, schools, YouthZone, and other community agencies.
The teams help families identify their strengths, needs, and develop a collaborative service plan. Service planning can include brainstorming options, troubleshooting challenges, connecting families to community resources and services or mental and behavioral health providers. This position works closely with the Collaborative Management Program Coordinator and Wraparound Facilitator at YouthZone. This position serves families throughout Garfield County (Parachute to Carbondale).

Administer, direct and oversee all clinical activities of the clinical and juvenile court programs. Supervises all clinical and case management personnel. Promotes clinical activities and positive public relations. Recommends and helps develop policies and procedures. Performs marketing and outreach activities.
